[ARCHIVED] Vaccine Administration by Appointment Only at FoundCare Health Center

Vaccine administration of the 1-dose Johnson & Johnson COVID-19 vaccine will be held this Saturday, March 27th, by appointment only for Village residents at the FoundCare health center located at 2330 South Congress Avenue in Palm Springs.

Appointment scheduling will be available online by visiting https://tinyurl.com/FoundCareAppointment.  If you are unable to schedule online, you may also call (561) 967-0365. Online registration is strongly encouraged.

Appointments will be scheduled on a first-come-first-serve basis to those who meet the required eligibility guidelines set forth by Governor Ron DeSantis. Those who are eligible to schedule include:

•             Persons 50 years of age and older;

•             Persons determined to be extremely vulnerable by a physician (form required)

•             Health care workers with direct patient contact

•             Long-term care facility residents and staff.
